(Please note: Returning students- Do not need to re submit their DD214 or Certificate of Eligibility for the GI Bill – unless there have been changes since last term on either one).
- DD214 – Member’s four or long form which includes discharge information.
- Print out from the https://hazlewood.tvc.texas.gov/students/Account/Login and/or Hazlewood transcripts are required from all institutions at which you have used Hazlewood since Fall 1995.
- Need Certificate of Eligibility (COE) or Benefits Letter from Dept Veteran Affairs (If you served after 09/11/2001 or receiving CH 31)
- Select a Program of study
- Tuition Account Summary or a copy of your CE sign up sheet completed
I am an Eligible Dependent of a Texas Veteran
(Have a parent or is the spouse of a veteran of the U. S. Armed Forces, Texas National Guard, or Texas Air National Guard who died as a result of service-related injuries or illness, is missing in action, or became totally disabled for purposes of employability as a result of service-related injury or illness)
- DD214 – Veteran’s Member four or long form which includes discharge information.
- Print out from the https://hazlewood.tvc.texas.gov/students/Account/Login and/or Hazlewood transcripts are required from all institutions at which you have used Hazlewood since Fall 1995.
- Need Certificate of Eligibility (COE) or Benefits Letter from Dept Veteran Affairs
- Birth Certificate or Marriage License to dependency status to veteran
- Select a Program of study
- Tuition Account Summary or a copy of your CE sign up sheet completed

- Dependent / Legacy Hazelwood application

- Need to Select a Program of study
- Print out from the https://hazlewood.tvc.texas.gov/students/Account/Login and/or Hazlewood transcripts are required from all institutions at which you have used Hazlewood since Fall 1995.
- Need Certificate of Eligibility (COE) or Benefits Letter from Dept Veteran Affairs (If veteran has served since 09/11/2001.)
- Tuition Account Summary or a copy of your CE sign up sheet completed
